Bachelor’s Level Care Coordinator Residential Services

Hiring Range: $24.94 - $30.55 Per Hour

Kitsap Mental Health Services is hiring a Care Coordinator to join our Residential Services team. You will utilize your clinical expertise to plan, develop, coordinate, and provide our clients with treatment, rehabilitation, and support services. As a key team member, you will also offer assigned clients education, family consultation, and crisis intervention services.

The Residential treatment setting is a 16-bed residential treatment setting. Client participation is voluntary, and people may stay for up to 30 days. RS team members provide a combination of supportive services designed to help people return to their families and communities equipped with the skills needed to manage life’s challenges.


Primary Responsibilities:

  • Provide care coordination services for an assigned group of clients, offering individual, group, family counseling, crisis intervention, and case management.
  • Develop, implement, evaluate, and revise treatment goals and plans in collaboration with the treatment team.
  • Educate and support clients' families and advocate for clients' rights and preferences, coordinating with community agencies as needed.
  • Assist clients in finding and effectively using primary care physicians and dentists, and develop health self-management skills.
  • Provide crisis intervention counseling and coordinate with crisis services when appropriate.
  • Document client progress according to established policies and procedures.
  • Participate in staff meetings and treatment planning review meetings.
  • Provide information to clients on substance use issues and coordinate treatment with internal and external substance use treatment providers.
  • Assist clients in gaining resources and education to support their autonomy and basic needs.
  • Monitor clients for LRA compliance and facilitate extensions with the CRT and court-involved personnel.
  • Perform any other responsibilities appropriate to the position and not specifically listed in the job description.

 

Minimum Qualifications:

EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field.

EXPERIENCE: Entry level (no prior related work experience)

LICENSURE: Agency Affiliated Counselor Registration. (Can be obtained after hire)

 

Preferred Qualifications:

EXPERIENCE: Experienced (minimum 2 years of job-related experience)

Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.
